I hereby authorize Gabe’s Professional Auto Solutions Inc. (herein after referred to as Gabe’s GPS), and its designated third-party providers to complete the repair work on my vehicle, as outlined above. I also authorize the purchase of parts and materials necessary for said repairs. I give Gabe’s GPS Employees and third party providers permission to operate the vehicle referenced above, on streets, highways or elsewhere for purposes of testing and inspection.

I understand that Gabe’s GPS is not responsible for loss or damage to my vehicle and/or articles left in the vehicle in the event of fire, theft, or any cause beyond our control. Please remove your personal belongings from the vehicle, including child safety seats, medications, firearms, sunglasses…etc. You will not be able to access your vehicle once the car has been received into repair production.
Vehicles towed or driven in and then deemed a Total Loss, or if the vehicle is moved to another facility for any reason by the customer or Insurance Company may be subject to storage, lot, debris, administrative, estimate &/or clean-up fees. Any labor performed on the vehicle, tear down for inspection, towing, storage…etc. must be paid before the vehicle leaves the premises of Gabe’s GPS.

I agree that if I CANCEL work authorization before work is complete, I must do so in writing and am responsible for paying all charges for parts and labor incurred up to the date of the cancelation notice.

I understand that my bill must be Paid In Full before my vehicle will be released to me. Gabe’s GPS accepts cash, credit cards, insurance checks and Zelle as form of payment. Prior written notice must be given if you so desire the return of the used &/or damaged parts.
I grant Limited Power of Attorney to Gabe’s GPS, authorizing them to endorse any checks received on behalf of the vehicle owner(s).

I understand that every effort will be made to complete my vehicle withing the discussed timeframe. However, I also understand that Gabe’s GPS cannot be held responsible for delays that occur as the result of parts availability, insurance company delays and requirements, additional damage discovered in the teardown process, weather conditions and any other unforeseen or uncontrollable event.

Once the final teardown begins, I understand that additional damage may be discovered. In this case, a supplemental claim will be submitted on my behalf to the Insurance Company and that this amount will be included in the final total. If this is not an insurance claim, I understand that I will be contacted for personal authorization if the total work to be performed is over 10% of the original estimate.

I understand that I will incur Storage Fee charges at $200 per day if I do not pick up my vehicle within 2 business days or receiving notification that my repairs are complete. I understand that the storage fees are not usually covered by insurance companies. These storage fees will be my sole responsibility.
